The goal of health IT is to make critical information for patient care easily accessible across multiple different entities and systems. POLST, which stands for Portable Orders for Life-Sustaining Treatment, is a medical orders form that captures a patient’s goals/wishes for their treatment in emergencies when they are incapacitated and unable to communicate. This can include Do-Not-Resuscitate orders and other instructions for care teams. Physicians and EMS providers are trained to consult POLST, but they need easy and systematic ways to access this information. Oregon has one of the only mandated-reporting POLST registries in the country, the Oregon POLST Registry, which serves as a source of truth for the must current, validated POLST form for patients that have one.
